Participation by invitation only

If you have received an invitation from MATHCOUNTS or your Competition Series coach about participating in the DoD STEM Alumni Survey, and you are 13 years old or older, please complete the webform below. Once completed, you will receive a follow-up email with the survey link and instructions. Students who provide all the required information and complete the survey, will be awarded a $5 Amazon gift card.

This invitation is first come, first served. Once the student quota has been filled, this webform will be closed.
